As with any operation associated with the Federal Government, there are a few paperwork items to get through. The following forms should be downloaded to your computer (Adobe Acrobat® is required - get it here if you don't already have it.). Fill out the required forms and submit them to the Executive Officer for processing.

RESOURCES:

ENROLLMENT FORMS FOR CADETS:

•  NSCADM 001 - CADET APPLICATION AND AGREEMENT
  Used to record pertinent personal Cadet information as well as standard releases.
•  NSCADM 004 - PARENTAL AGREEMENT
  Used to request assistance from Cadet parents with unit operations and support.
•  NSCADM 007 - CADET ENROLLMENT/REGISTRATION FORM
  Used to submit enrollments, re-enrollments and transfers of NSCC and NLCC Cadets.
•  NSCADM 020 - REPORT OF MEDICAL HISTORY FORM
  Used to obtain personal medical history on NSCC/NLCC Cadets.
•  NSCADM 021 - REPORT OF MEDICAL EXAMINATION FORM
  Used to record personal medical examination information on NSCC/NLCC Cadets.

ENROLLMENT FORMS FOR OFFICERS, ADULT LEADERS, AND MIDSHIPMEN:

•  NSCADM 003 - OFFICER/INSTRUCTOR/MIDSHIPMAN APPLICATION
  Used to record pertinent personal Officer/Instructor/Midshipman information as well as standard releases and privacy statement.
•  NSCADM 005 - OFFICER VOLUNTEER QUESTIONNAIRE
  Used to obtain personal references of potential adult volunteers. A minimum of three references must be used (three separate NSCADM 005 forms).
•  NSCADM 020 - REPORT OF MEDICAL HISTORY FORM
  Used to obtain personal medical history on NSCC/NLCC Cadets.
•  NSCADM 021 - REPORT OF MEDICAL EXAMINATION FORM
  Used to record personal medical examination information on NSCC/NLCC Cadets.
•  NSCADM 006 - ADULT LEADER ENROLLMENT/REGISTRATION FORM
  Used to submit enrollments, re-enrollments and transfers of NSCC Officers, Instructors, and Midshipmen.

UNIT MANAGEMENT FORMS:

•  NSCADM 010 - RECORD OF AWARDS
  Used to record ribbon awards, appurtenances, badges/breast insignia, community service, color guard service, and any subsequent awards.
•  NSCADM 009 - RECORD OF ADVANCEMENT
  Used to record completion dates for advanced training, correspondence courses, promotions.
•  NSCSUP 002 - SURPLUS UNIFORM ORDER FORM
